New Patient Intake and Treatment Plan - Adult ( 21 and over)

Required for New Patients. We will review your Intake Form together, and discuss options, tests, and formulate a treatment plan. If it’s your first visit, this is for you.

FOLLOW-UP

ADULT (21 & Over)

Follow Up and Test Review - Adult ( 21 and over) 

We will review test results, and revise treatment plans as necessary. We recommend scheduling Follow Up appointments for every 4-6 weeks, or as needed. If you’ve seen Dr. Hennie before, this is for you.
